Thank you for your support of SCOK in our commitment to provide services to people services to people 60 and older and to make Kodiak Island a safe and good place to age.

If you would rather apply in person or via mail and pay via check, cash or credit card, you may print an application form and bring or mail it to the Senior Center at 302 Erskine Ave Kodiak, AK 99615.

If you chose family, please list the other people in your family who are part of your membership.
